University of Rochester Medicine, its divisions (including, but not limited to Strong Memorial Hospital, Golisano Children’s Hospital, University of Rochester Medical Faculty Group, Eastman Dental Center, Wilmot Cancer Institute, Pluta Cancer Center, Flaum Eye Institute) and its affiliates (including, but not limited to Highland Hospital, FF Thompson Hospital, Noyes Memorial Hospital, St. James Hospital, Jones Memorial Hospital, UR Medicine Home Care, Geneva General Hospital, Soldiers and Sailors Hospital, and their affiliates), doctors and clinics, and other health care providers that license Epic from University of Rochester Medicine through the Epic Community Connect program (hereafter individually or collectively referred to for convenience as "Provider") are pleased to offer you access to your medical information via a secure web-based version of our primary electronic medical record system (Epic) called MyChart. Please note that there may be records stored in other systems that cannot be viewed through MyChart. MyChart also provides a secure mechanism to communicate with providers and pay for services.
By requesting access to and continuing to use MyChart, you are confirming that you have read, understand and agree to these Terms & Conditions. These Terms & Conditions outline our practices and our sensitivity to your right to privacy. Provider does not guarantee that you will be able to access MyChart, its availability, or content completeness or accuracy, and reserves the right to revoke MyChart access at any time for any reason. In the event you wish to withdraw your agreement to these Terms & Conditions, you should discontinue use of MyChart and may deactivate your account. You may continue to use alternative methods of communication for contacting your health care provider at any time, regardless of whether MyChart is available.
NEVER USE MYCHART FOR EMERGENCY OR OTHER URGENT MEDICAL ISSUES. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY OR NEED URGENT MEDICAL CARE, CALL 911, GO TO YOUR LOCAL EMERGENCY ROOM, AND/OR CALL YOUR PHYSICIAN.
PROXY MYCHART ACCESS MyChart includes a feature allowing most patients to authorize other individuals to have access to their online health information or revoke such access via the MyChart Sharing Hub (referred to as "proxy access"). In addition, Provider may grant certain other individuals (e.g., parent or legal guardian of a minor, legal surrogate of an adult or HIPAA personal representative) proxy access to a patient’s MyChart. If you are accessing someone else's MyChart information, you agree to be bound by these Terms and Conditions. If a proxy’s legal relationship with the patient changes (e.g., a legal surrogate’s authority is revoked), the proxy agrees to inform Provider immediately and desist from further proxy access of MyChart. Provider reserves the right to revoke proxy access at any time for any reason. Proxy access is subject to federal and state privacy, security and confidentiality laws.
I understand that proxy access may include the disclosure of highly sensitive information, including, but not limited to, information relating to treatment for a SUBSTANCE USE DISORDER, GENETIC TESTING, PSYCHIATRIC CARE, REPRODUCTIVE CARE, GENDER AFFIRMING CARE and/or HIV RELATED INFORMATION. In the event your medical record contains information you wish to remain confidential, you should not authorize proxy access or revoke such access via the MyChart Sharing Hub. If MyChart contains records of treatment for a Substance Use Disorder, the proxy should be aware that federal regulations at 42 CFR Part 2 prohibits unauthorized use or disclosure of such records.
Electronic Communications MyChart enables you to exchange electronic messages and other information with your health care providers. Such communications may be routed to an appropriate individual within your health care provider’s department and/or practice. Therefore, individuals other than your personal health care provider may review your medical record information as required to address your request. If your communication contains sensitive information that you want shared only with your personal health care provider, you should contact such provider directly and not through MyChart. It is your responsibility to ensure complete and accurate information is provided and to regularly monitor your MyChart inbox and your personal e-mail for message notifications. All communications sent through MyChart will become part of your permanent medical record and may be used and disclosed as permitted by law.
Provider will try to provide a timely response to electronic inquiries, but you should allow at least two business days for a response. Delays may occur based upon message volume, availability of staff, the complexity of the patient's condition, or technology disruptions. Therefore, Provider cannot guarantee that messages will be responded to.
Web Site Links MyChart may offer links to related medical Web sites not managed by Provider. These Web site link(s) are for a patient's informational purposes only. Provider does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.
E-mail Privacy Patients who are users of MyChart should be aware that they will be notified via e-mail when there is new medical information to be viewed on MyChart. This means that any person with access to a patient's e-mail will be able to see this notification. This could include members of your family or anyone else that can access your e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address or using MyChart.
Security Considerations Information from your medical record that is accessible through MyChart is intended for your access only (subject to the discussion under “Proxy Access”). You will have a unique username and password for MyChart and, if applicable, a pin for MyChart Bedside, which will provide you with such access, which are crucial to protecting the security of such medical record information. You should select a password or pin that is confidential, and it should not be shared with anyone. In the event you believe your password or pin has been compromised, you may change it anytime through MyChart.
Provider will use certain security measures within the scope of MyChart such as verification of identify through the use of activation codes and tracking of activity within the account. However, once you access your medical information through MyChart, you are solely responsible for maintaining the security of such information on your end as well as the information required to access such information (i.e., username and password or pin). Specifically, you are responsible for the results or consequences of your communications regarding, or the sharing of, information accessed via MyChart. When using MyChart, information will be transmitted over an Internet medium that is beyond the control of Provider and its contractors. You are responsible for ensuring that the computer system(s) you use to access MyChart have the appropriate security controls enabled.
Mobile Device Platform In addition to viewing MyChart from your computer or laptop, you can also access MyChart via a mobile device such as an iPhone or Android. The mobile application has most, but NOT ALL, of the MyChart functionality. You will still be able to view test results, medications, view/send messages, but you will not be able to request a medication refill or request other changes to your chart. If you elect to view MyChart via a mobile device, such use via a mobile device is subject to these Terms and Conditions.

Prohibited Actions You agree that you will not upload or transmit any communications or content of any type (including secure messaging) that may infringe upon, misappropriate or violate any rights of any party and that your use of MyChart will not include any of the following:
Information Does Not Constitute Medical Advice Provider assumes no responsibility for how you use the information you obtain via MyChart. You should consult with a physician or other health care provider regarding your own condition and how MyChart content may or may not apply to you. Neither MyChart nor any other application available through MyChart is a substitute for appropriate and timely consultation with your health care provider. You should never change or stop any course of treatment prescribed by your health care provider without first consulting with such health care provider.
Accuracy and Integrity of Information Although Provider attempts to ensure the integrity of MyChart, it makes no guarantees whatsoever as to the correctness or accuracy of its content. It is possible that it could include typographical errors, inaccuracies or other errors. If an inaccuracy arises, please inform Provider so that it can be corrected. Information contained within MyChart may be changed or updated without notice.
MYCHART BEDSIDE MyChart Bedside may be accessed on a personal device or through a tablet provided by Provider. Acceptance of the tablet means that you agree to take care of the tablet and charger and prevent them from being damaged while they are in your possession. The tablet and charger remain the property of Provider at all times and must remain in your room during your admission. You may be asked to return the tablet and charger at any time. Please notify Provider immediately if the tablet is lost or stolen and return the tablet and charger in good working condition at the conclusion of your hospitalization.
You control who has access to the tablet. When utilizing MyChart Bedside, you will be prompted to follow the directions on the tablet to set up a PIN. You are responsible for deciding who, if anyone, you share your PIN with. Although your medical information is not stored directly on the tablet, failure to establish a PIN will allow anyone with physical access to the tablet to access your medical information during your hospital admission.
Please note that the tablet will allow limited access to the internet, primarily focused on a limited number of education and health-related Web sites. However, Provider cannot assume liability for the accuracy or completeness of any content contained on these sites as they are maintained by third parties.
MyChart Bedside is not a substitute for medical care and should not be used for emergencies or urgent matters. In an emergency or urgent situation, please contact your nurse or physician directly. MyChart Bedside is subject to all the Terms and Conditions set forth herein. Failure to comply with these Terms and Conditions and/or abuse of the tablet or the MyChart Bedside application, such as excessive messaging or recording others without their permission, may cause your MyChart Bedside access and use of the tablet to be revoked without prior notice. Provider may terminate your access to MyChart Bedside at any time for any reason.
TEXT MESSAGING (SMS) When you sign up for text messages from Provider, you are signing up to receive text messages related to your relationship with Provider related to your visits, MyChart account, one-time passcode, billing notifications, prescription reminders, and care management.
You can opt-out of SMS messages by texting STOP to 58263. Your opt-out request will generate one final message confirming that you have been unsubscribed. You will no longer receive SMS messages from the short code you opted out from. If you want to join again, sign up using MyChart or text HELP to 58263 for instructions.

USE OF THIRD-PARTY SERVICES You may choose to use other services provided by third parties in conjunction with your use of MyChart. Such services may include, but are not limited to, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor Provider have control over the contents of these services, and neither Epic nor Provider accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information and may be used and/or stored by a third party. If you decide to access any such services in conjunction with your use of MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services. Provider reserves the right to disable any 3rd party application if needed.
RESEARCH Within MyChart you may receive notifications or messages from researchers about studies you may be eligible for. To learn more, log in and go to your Research Studies page. If you do not want to receive these messages, you must log in and indicate that you do not want to be contacted about research studies through MyChart. If you do want to be contacted, you can create a profile to select areas of research you are interested in, see what new opportunities you may be eligible for and what studies you are enrolled in. If you do not make a selection, we will assume that you are willing to be contacted.
EPIC COMMUNITY CONNECT To facilitate the coordination of care, the University of Rochester Medicine has provided certain community providers the ability to use Epic and its MyChart system for their own patients. These community providers are independent practices and providers and are not a part of the University of Rochester or its affiliates. While the University of Rochester Medicine offers this capability to the community providers patients as a convenience, the University of Rochester Medicine and its affiliates are not responsible, in any way, for these community practices or any of their activities.
PRIVACY POLICIES Privacy Of Your Healthcare Information
The privacy of MyChart users' healthcare information is protected by state and federal laws including the Health Insurance Portability and Accountability Act (HIPAA). How we protect your messaging and medical information on MyChart To protect your privacy, Provider’s employees receive privacy training and must adhere to HIPAA and other security requirements that include accessing the minimum amount of patient information necessary for that patient’s care. Provider’s privacy officers regularly audit such activity to ensure compliance.
DISCLAIMER OF LIABILITY PROVIDER EXPRESSLY DISCLAIMS ANY AND ALL WARRANTIES CONCERNING MYCHART AND ITS USE BY YOU, WHETHER EXPRESS OR IMPLIED, STATUTORY OR OTHERWISE, AND PROVIDES THE MYCHART ONLY ON AN “AS IS” AND “AS AVAILABLE” BASIS. Under no circumstances shall Provider be liable for incidental, special, indirect or consequential damages or loss of profits, interruption of business, or related expenses which may arise from your use of or inability to use MyChart and other related services therein.
IN ADDITION, PROVIDER DOES NOT GUARANTEE THAT USE OF MYCHART WILL BE FREE FROM UNAVAILABILITY OF INFORMATION, SERVICE DISRUPTIONS, INTENTIONAL OR INADVERTENT INTRUSIONS, VIRUSES OR MALWARE OR OTHER MALICIOUS SOFTWARE. YOU ACKNOWLEDGE THAT YOU ARE SOLELY RESPONSIBLE FOR ENSURING THAT YOUR COMPUTER OR ELECTRONIC DEVICE HAS ADEQUATE PROTECTIONS AGAINST ANY OF THE FOREGOING.
